As others have said:
  1. Close Calibre. There should only be your original, pinned icon now.
  2. Unpin this icon. You should now have none.
  3. Open Calibre using desktop icon, or start menu. There should be one icon in taskbar.
  4. Pin this icon.

If this still does not work, reboot computer between 2 and 3.
