I had the same trouble to get it to work. The sequence is as follows:
- close calibre
- unpin the (old) icon
- run calibre from the start menu (or from an icon on the desktop)
- a single calibre icon appears on the taskbar
- now right-click on that single calibre icon on the taskbar
- pin it
- done!

From now on everytime you use the icon on the taskbar, no 2nd icon will appear.
