I followed you instructions and it worked well. I did read the help instructions and I think they are fine. After configuring the columns etc, I instead thought that I had to follow the instructions in Change Reading Status in device library part. Percentage reading more sounds like "reading status" than "bookmarks".
To me, a "bookmark" was something I put into a paper book to remember where I was up to. When I created this function, I wanted to record the position in the book so it could be restored. Hence I used "bookmark". But, it seems to be easier to talk about this as the "reading status".

That could happen for two reasons I can think of. Firstly, the copy of the book you are sending now is not the same as the copy you were reading before. The location stored depends on the structure of the book. Changing this will mean the location is no longer valid. As will changing the formatting in the book. Also, the location is different for epubs and kepubs and there is no way to convert between them. So, if the restored location doesn't point to location in the book, it will probably open to the cover.

The other reason is if you don't store all the values. The "Current Reading Location" column is the only important one for storing the location. If you don't store and restore this, the library on the device might that it is being read, but it will open to the cover.
