Sorry to comment on an old post, but I just had this issue and found a solution.

When I received this error, I clicked Send to Apple, which shows all the metadata for the package and the error message at the bottom. I copied all of that, pasted it into Word and then searched for file_name, which was the field causing issues. Sure enough, I found that I had mistakenly named the cover image "Ipad..jpg" with 2 periods. I removed one of the periods and voila everything was fine.

So the main point is that you can figure out where the error is by searching the error log for file_name and making sure there aren't any errors in the name of the item in question.
