I added a TOC to a book I had by creating some h1 headings and then generating a TOC from the headings. I then inserted the TOC inline and moved it just below the cover image.

Using View in Calibre, everything looked great. The TOC showed up right after the Cover and I could move to specific headings by clicking on them.

So I transferred the epub to my Kobo Glo HD. When I open the book I see my cover and next page shows blank and 100% read. I set the book to unread but same thing happens.

The rest of the epub is there. I can navigate to the TOC and use it to move within the epub but next page from cover goes to 100% read.

Any ideas??
