Version 1.0.35 was just uploaded. It will automatically become available in Calibre at :32 after the hour, but it can be manually downloaded from the OP now, and then manually installed via Preferences > Plugins > Add from File.
I tested your CSV with the new version, and it worked fine except for the fact that the paths of the file attachments do not exist on my PC.
