The Quality Check plugin passes the file in questioned so I can't pin point all the other broken image pages for the other books with that. I did find out that need to delete the titlepage.xhtml and make calibre recreate it so that the conversion bug doesn't happen.

I think the only solution is to strip delete the cover/titlepages and make calibre recreate them. Is there a plugin for that?

I take that back. Some broken image link files didn't work when i delete/recreated the cover page. I'm just going to give up about this silly bug.
