This was my main point but I also made a side note saying that I wonder why Amazon staff apparently reprocess their books (and add a better TOC as stated by another poster) instead of telling publishers to do a better job...
I know at least 50 author/publishers that got letters to either fix their TOC or their books would be pulled.
