I just use a simple Notepad document I used to use for the Kobo contest codes.

I copy & paste the code on it with the date when I redeem points for a code.

When I buy a book I cut & paste the earliest code from the document to the website, but don't save changes unless the transaction goes through to be sure the code worked (so I can hit undo if it doesn't). Once the transaction is completed, then I erase the date and the next code is now on top for the next time.

I also keep all of the reward code confirmation emails.

It looks like this:
