I checked some books on amazon.com and the "Page Flip: Enabled" matched "Enhanced Typesetting: Enabled" in that either both or neither was found in the small assortment of books I checked. This makes the association with KFX very likely.

I updated the Kindle app on my iPad to the new 5.0 release that supports page flip. I then took a public domain EPUB and converted it to both AZK and KFX and sideloaded these files. The AZK version did not allow page flip, but the sizeloaded KFX did allow it. More confirmation of the association of page flip with the KFX renderer.

ETA: Although the side loaded KFX allows page flip to be activated the thumbnail pages are mostly blank. I need to do more testing.
