Run Library Maintenance (both phases) and resolve the reports.
BUT Before that, have the OS check (and repair) the drive (folders)
Unless an active Calibre (Add/rename) operation was interrupted, those results could not occur on a standard drive. Changes are done almost immediately (Only that OPF may lag if part of a bulk change. That is a metadata backup)
