I just don't understand the point I suppose. If a book does not have a cover image, that added file is still going to just be the first page, isn't it? Unless some devices crash or don't work properly without it, in which case I understand the necessity of adding it in.

Yes, this is to improve compatibility with some readers or apps. Some are looking for an image in the book that is marked as the cover. The Kobo ereaders handle epubs by rendering the first page. But, for the kepubs, it is using an image in the book. Which image is defined in the OPF in a different way than defined here.
