I am confused. Adding a book to calibre does not modify the book in anyway. So unless you manually update metadata in the in-library copy using either the embed metadata tool or the edit book feature, you are not going to have the cover file added.
And I dont see what the problem with the location of cover.jpg is. Since cover.xhtml does not reference cover.jpg in any way.
