I'm old enough to remember going to the doctor, getting a bill, and writing a check.
Now, the first question you get asked is about insurance coverage.
A former employer changed health insurers in the interests of saving costs. The change was
not for the better in terms of coverage. I really wanted to decline coverage. I'd have done better to take the money deducted from my paycheck and stick it into an interest bearing account, and use that to cover medical expenses, but that wasn't an option. (An old friend who makes his living on investments is doing precisely that.)
But the insurance companies aren't really the villains of the piece. They price to cover their costs, and the problem is that the underlying costs of providing health care have soared. I haven't seen anyone propose plausible solutions for reducing what it costs to provide health care in the first place.
