On the Kindle...

It is built from the semantic ToC.

In the calibre E-Book Viewer it is the navigation menu on the left.
You can use the ToC Editor to modify it.
The Kindle will allow you to skip from location to location in the ToC, but it has to be properly linked.

The HTML Table of Contents is nothing but an HTML file with clickable links. It has no special meaning whatsoever... although I am unsure if calibre has an option to suppress it when converting.
