I think I have the same problem on jb 5.6.5 pw3.
Fixing this by rebuilding the library from folders in Librarian Sync instead of updating from Calibre plugin. Sometimes I need to use the magnifier icon to launch KUAL, when it does not show up in the listing.

PS: works for me because I don't mind having author_sort == category name

Edit: Do you have some very large books, i.e. omnibus with lots and lots of chapters? I know I do, like 21-book or hundredfold-chapters compilations) Could it be a problem with indexing those?
