That's probably the last time I wrote the full details like that. And looking at it, I missed a tool. There is also the Annotation Exporter by Toxaris. It has an option to create a HTML file for each book with annotations, so it might be closest to what you want.

Also, I don't think any of these methods really care if there is a note attached to the hightlighted text in book. The database table used for bookmarks, highlights and annotations is the same. The differences are the columns filled in. All the methods pull the full details and format the output as appropriate.
