I ended up laying out the heirarchy that I wanted in an SQL script to add it directly to the Mantano database. Whenever I buy a new device, I pull the database back to my PC, run the script, and push the database back. I still have to add each book to its collection when I import it from Calibre Companion, but I really like the interface once that's done.
Given that you're using Calibre Companion there's no real reason why you need to see the Bookari "home screen" at all, is there? Open book from either CC Book Details or CC widget or CC BookList/CoverGrid - Choose Bookari as the app to read with and you're directly into the book, not the Bookari home screen.

Although I don't use it myself I believe CC has a rudimentary ability to nest Collections - possibly only down to one level, though.
