Many Random House (Ballantine, Del Rey) often has a generic cover. Harper Collins does as well. See the attachment for an example

I have *many* books published by the Random House and HarperCollins groups that originally came with a generic cover.
Often the book shows up in stores with a cover, but downloads without one.
