Let me see if I get your sequence right:
1. Select a new Mark file
2. After selecting, immediately edit the mark file
3. The old Mark file is opened for editing
Is this correct? If so, it might not detect the change of the file. It would also be strange, but it might help me pinpoint the idea. Even so, it still should make a difference between 1.20.3 and 1.20.4. Literally nothing changed there.

Yes - that's it
Actually the first time I just switched from MarkRestoration to MarkFinance and used it - but finance words weren't marked so I went back to settings (assuming I had mis-selected) but MarkFinance was selected, but when I went into Edit I got the MarkRestoration list.
As I said I checked the files and they appeared to be OK.
I assume the path of the current list is in in the config file - maybe that's not being updated. What should I check for ?
FWIW - the calibre DOCX to EPUB Conversion picks up the Highlights, which is a handy and unexpected bonus
