Just like jhowell did. Run through previewer, convert with a script from kdf to kfx, sideload. If you can live with the limitations. Cover doesn't show up on homescreen, and the cover in book shows up as an image with location on the bottom. In a real kfx book the cover fills the screen and keeping the aspect ratio correct (e.g. white bars on left and right for skinny covers). Advantage? Only one file that needs to be sideloaded. I could live with that.
Edit: oops my bad. All kfx books that I have show the cover incorrect with page number on bottom (all the books were originally released before kfx). The one exception is
Little Sister (February Kindle First book) that shows the cover in full screen without page number in bottom. Either the automatic conversion of old books to kfx is faulty, or Amazon changed something when processing new releases.
