I've had the requests get lost and had to resend. This always seems to be a bigger problem when they had a lot of 100% rewards going on, especially if purchased the first day they were available (I suspect the person's email box simply was flooded). If I don't get it by near the end of the next business day, I always resend the request.
I've also had their email to me get lost (or eaten by an ISP spam filter), so the credit was there, but I didn't get a reply.
