No surprises there; CoolReader is notoriously bad at respecting the formatting that's in the book. It just imposes whatever formatting it decides to use (based on criteria such as "is there an 'R' in the month?"). Try using BlueFire reader; it's based on Adobe ADE and should respect your formatting.
