Using an AZW3 caused the incorrect highlights like you mentioned, but for me I was able to set the offset to -16 and everything aligned properly again (for the 3 books I tried!). I have no clue where this offset is coming from...
Try -16 and see if it works for you.
It seems to work perfectly with -16. The highlights are still misplaced in the x-ray menu, but in the actual text it works.
