I purposely defocus my eyes --- and then do it again and again if I slip and start thinking about something and refocus them. When falling asleep during a lecture (er, sermon, actually) I had the hardest time keeping my eyes from defocussing, leading to sleep .... so I purposely reverse the process to GO to sleep.

But yeah, my CPAP low noise helps too.
