In a "real" book, I don't know. But, if I converted a random Wikipedia page, there would probably be one. The problem is that it is a valid thing to happen, so it has to be considered.

The real thing is that we are way beyond the point that a footnote link in an ebook, or any other electronic form, should look and behave like in a paper book. And the book authors/creators should be doing something different. And the software creators should be as well.
