Are we talking about an html toc or an ncx toc? I would assume the page-progression-direction="rtl" would affect (x)html in the spine, but I've no idea what reading systems might do with the ncx. I admit, this is not something I have any practical experience with. I would also suspect that rtl support is likely to vary wildly from reading system to reading system.
