I'm not sure you'll be able to diagnose the problem without going into the code, but the code generated from InDesign can be pretty noisy, so you may not be able to find it even then.

You might consider building a separate document version that has very minimal formatting, be exceptionally meticulous and picky about the formatting it has, and generate an ePub from that. Perhaps in pieces, to help isolate the problem.
