Problem with 'alexa' response was not due to room noise (there was none, apart from what the device was reading out). We had to go right up to it and TALK VERY LOUDLY, whereas normally it would readily respond to 'Alexa, pause' from anywhere in the room.

Echo has visual volume indicator, you can adjust it with 'alexa, increase volume to 7' (scale of 10), use the remote (if you have one), the remote Alexa app on a mobile device, use a web browser on a computer on the same local network, or by turning the ring on the top of the device as it happens to be a volume knob (clockwise to increase volume, counter-clockwise to decrease). If the indicator is white 360 degrees, it is at max volume.
