Thank you sooo much! It was driving me absolutely insane trying to nail this down. It happens when saving after running EpubCheck. It doesn't happen if you don't save, and it doesn't seem to happen when saving after running the Flightcrew (or any other) plugin. Now that we know "what" we can figure out "why" and how to fix it!
EDIT: ...and done. The Epubcheck plugin writes a new mimetype file to Sigil's scratch directory (bk._w.ebook_root). Suggested a fix in the plugin's thread.
Different things (including saving with calibre's editor) will apparently eliminate the extra mimetype file. I think we should probably try to make Sigil be able to self-correct something like this as well (and possibly figuring out a way to make the contents of bk._w.ebook_root downright inviolable by plugins).
I'd be very surprised if this wasn't the problem with your books @exaltedwombat.
