If it is outside the one week, you have to engage CS and have a reason (usually not just "I hated this book.") So, if there are formatting issues, readability issues or a lot of typos--things like that, you can still get a refund. The week period is automated. You can just check a box "typos" or "format" or whatever and it refunds.

Some people have reported that they gotten refunds just because they didn't enjoy the book, but circumstances are a bit vague there. It could depend on a number of things Amazon tracks (like how many things you've returned, or who you talk to or...who knows?)

If the book has been updated because of a known issue, you can get it replaced or refunded at any time. An author I follow had a book that was pre-order, but Amazon actually messed up the upload or delivery--when it went live, only the sample was sent instead of the whole book. It was a bit of a mess and the author had to work to straighten it out (By that time, several refunds had been issued, the author had been blamed...and even with notification, some people were leaving bad reviews for ages because they either ignored the notification or didn't want it because they didn't realize what was going on). There's been a couple of instances where something just doesn't work and it can take a while to work out. (Amazon has done this with audio books more than once. They take the audio that is loaded and create multiple tracks of their own liking--only in some cases they don't get the order of the tracks right and it's a big, fat mess.) So be aware you can get a refund for something that is a mess even if it is after the 7 days depending on the issues.
