Typically numeric, but in theory it could be any sort of symbol or foreign-language unicode.
Is it more typical in the NCX than in other parts of the EPUB?

Well, yeah -- Meatgrinder would always be my first guess.
It's an automatic conversion, and we don't know how it works. For that matter,
any automatic conversion is risky.
