Well, what does that file look like? And if you edit the book with calibre's Editor and run Check Book, what warnings or errors does it show?

P.S. Stop exporting HTML in Word. calibre can convert DOCX --> EPUB, or open DOCX directly in the editor and save as EPUB (avoids CSS flattening) and does a far better job than the garbage that Word calls HTML exporting.
