I do not know if I use a bad Calibre setting but when I convert the file I posted, if in the Visualization panel I ask to delete blank lines, I lose the structure of the index.

The "table of contents' at the top of your book is normal style text that includes blank lines. I suggest you use Word/LibreOffice to create a navigable Table of Contents which calibre will convert to a navigable EPUB ToC (I think
).
