When calibre sends a book to CC, calibre updates the metadata and the cover inside the book then hands the book to CC. CC stores the book unchanged on the device. Because of the metadata rewrite the cover in the book stored by CC can be different from the cover in the book stored by calibre.

As I said above, we don't know whether there is a problem with the book or a problem with FBReader. Do you have another epub reader app that you can try? Does FBReader have a setting for a cache directory where it stores cover images?
I don't have another epub reader but could try one you suggest? FBReader has a setting to control many items but nothing relevant to this. Thanks for your fast reply- I'm off to find FB Reader support.
