One solution, if I understand correctly:
Use a regex only for the selected files: cap1_en, etc. (Selected in the file browser).

Code:
Search: <sup class="bkm">(\d+)</sup>

Replace: <a data-index="\1es"><sup class="bkm">\1</sup></a>
