It may be confused if you used more than 1 OS on the Library
Windows is case insensitive. Linux can create john smith, John smith, John Smith folders Windows can not deal with 3 folders with the same name (to it)

Thank you. Your suggestions pointed me in the right direction. After checking the library I could see that two of the twelve books in the author's folder showed up as 'Extra Titles'. I deleted just those two books from Calibre and the rest of the contents of the folder was
not deleted. I then added the books from a backup and everything seems to be back to normal.
What's odd is that checking the library showed me that I have 40 to 50 books that show up as 'Extra Titles'. I have never manipulated the library folder outside of Calibre, but the database got corrupted anyway. Looks like I need to check the library occasionally so I don't end up with a bunch of titles to fix.
