I've seen the second and reported it to Kobo with a sample book. The others all sound like symptoms of the same underlying problem. I'll test and add them to my report.

As to when it started, I agree that 3.16.0 was OK and 3.18.0 is the first time I noticed it. From something that Kobo said about changes on the beta list, it wouldn't surprise me if it started with 3.16.10. I'm almost willing to try 3.17.3, but there's no way I'm going back to 3.16.10.
