I've noticed that the sound doesn't always play, but I simply assume that was from listening to too much rock and roll with headphones when I was a teenager.

I think that some of the notification problems might be a level of confusion as to what is the active device at a given time. I have gotten notifications on my iPhone, Apple Watch, iPads and iMac. I seem to be a lot more likely to get the appropriate notifications when I'm away from home and have my iPads set to airplane mode. That means that the only device that is in range of my iPhone is my Apple Watch. If the iPhone is currently active, I usually get the notification there, if it's not, then I get the notification on my Apple Watch. But sometimes, I don't get any notification at all. It's odd.
