More: I had 1.13.0 work after restart and install, then upon shutting Word down and running it again, it was inactive again. So it's not certain that it worked when I did a W8.1->W10 direct upgrade. That might have been the same kind of temporary thing, since I only checked that it was there; I didn't do any work or restart Word in the couple of days before I did a clean install of Windows 10. Also, the January 2014 .exe version does show up on the ribbon, even after Word closure and restart, but most of the functions (Settings, etc.) don't work. So unfortunately that's not a temporary solution for me.
The add-in works fine with Windows 10, even after a clean install. That is not the issue. It *might* be an issue with the settings file. Can you check what is in the directory %appdata%\Toxaris? There should be a file called 'ePUBTools.cfg'.

Have you checked that .Net Framework 4 is installed?
