If I am not mistaken, the current Word version in Office 365 is Word 2015, right? It might be that something has changed in that version with regards to add-ins/VSTO. I will try to check that out. I do not have version 2015, so I am unable to check.

Is the add-in enabled? Have you checked that .Net Framework 4 is installed?

The screenshot looks normal to me though.
