Was this a book that you loaded via USB, or is it a book bought from Amazon? If it's the latter, are you sure that you're not seeing the version of the book that's in the Cloud?
If you did sideload it, check to see if anything's got stuck indexing - that can affect the refresh of the home screen. Go to the home screen, tap the "Search" button, and enter a string of random characters. On the results page, if the "Text in Books" tab isn't greyed out, select it and see if it says that there are books waiting to be indexed.
