Are you adding the book to a collection on the device itself? The calibre / Kobo integration works on the assumption that calibre is the 'master' for all collection management on the Kobo and will update the device to match calibre's list of collections.
I think there is a device driver option to prevent this, but am currently not at my PC and can't check.
