I think it's the Adobe render software which doesn't support font-variant, rather than Kobo. It's been that way on every epub reader (Sony, Pocketbook, Kobo) I've owned in the last 6 years. We live in hope
If it matters a lot to you it's not particularly difficult to sideload a true small-caps font on the Kobo and reference it from standard epubs.
