Ok had a look at 2 books I know I got 1 as AZW3 and 1 as Mobi

both have

<reference href="titlepage.xhtml" title="Cover" type="cover"/> in content.opf guide section.

So cant be that for these. Will have a check through and play around with them and see what happens when I change other bits.
