If no cover is declared (semantics), Calibre assumes the first image found is the cover.

If you use the E book-Editor, you can easily assign proper cover status to :
1) The Image file (eg foo.jpg)
2)The Cover Page (eg mycover.xhtml)

Both need to be done for it to be bullet proof (other tools may replace undeclared files)

Note that only one semantic can be assigned to a file (precludes 1 monster file). Cover, titlepage, TOC (if used), Text (Start) need (should) to be separate. All others semantics are mostly optional, but must be isolated if used.

Optionally, you can hand edit these into the Manifest of Content.opf of your book.
