'Duplicates' happen when the (Author+Title+Identifier) metadata (that hits Calibre) is the same.

If you use the Filename Method (Preferences: adding books), the TEMPLATE must be able resolve those from the filename (and end up with a unique pair)

If you use the internal (to file), It has to be there and it has to also be unique
