You need to crop out the top and bottom margins with the -mt and -mb options. I can't really help with starting the pages at certain places and adding bookmarks for them, though. There's not a way to do that. This is the best I can do, which will automatically find the three columns (on the first page, at least--you didn't send any other pages):
k2pdfopt -col 4 -mt 3.3 -mb .475 inpdfviewer.pdf
