I suspect that means the first time you tried was before ejecting the device and the second time on the next connection. If so, that is expected and how it works.

If you believe you can reproduce it, run calibre in debug mode and post the log in the Kobo Utilities plugin thread. I will look at it. But, it should work. Changing the save template isn't that common, but fixing the title or author is. Or putting the book the SD card or a different device or after a factory reset. Restoring the reading position works for all of these. I have done all of these except the SD card one in the last few weeks with no problems.
