That happens when book matching fails. Usually that happens when the metadata in the book does not match the metadata in calibre, often because people use plugboards to change the metadata but sometimes because the format doesn't contain enough metadata to match anything (think txt and some pdfs).

Regarding the one-at-a-time: the "change folder location" suggestion should avoid that, assuming that any book file left behind because it isn't in CC's database is no longer of interest.
